• This Mukhyamantri Lok Sevak Arogya Yojana will replace the current medical bill reimbursement process of the state government.
• Under the scheme, beneficiaries can register themselves and their dependents on their own or at the district kiosk.
• Advance payments under the scheme can be accepted from the recipients.
• The CGHS-approved rate under the scheme will be used to pay the expenses.
• Under the scheme, end-to-end claim processing will be monitored and taken care of by IT
• Under the scheme, advance payment will be given directly to the hospitals.

• To apply under this scheme, the eligible beneficiary must first visit the official website of the scheme from the link given

• Then you will have to click on the Click Here button in the Beneficiary Login/Registration option on the home page itself.
• Then you have to fill in your employment type, PAN/PPAN/GPF number, and mobile number.

• Then you will have to click on the Proceed button and an OTP will come on the mobile number filled by you, which you will have to fill here and click on the Proceed button.

• Then you will get the block profile of beneficiary details on the new page, which you will have to fill. Due to this, you will have to click on the Next button.

• Then you have to fill in the type of beneficiary, his name, gender, and date of birth on the new page.

• Then on the new page, you will have to fill in the beneficiary address information like mobile number, email ID, permanent address, district, and pin code.

• Then you have to fill in the employment information like GPF/NPS number, employee code, retirement date and grade, etc.
• Then you will have to provide DDO information on the new page.

• Here you have to fill in Department, District, Treasury DDO, etc., and click on the Save button.
• Then you have to add the nominee/dependents on the new page.
• For this, first of all, you will have to fill in the dependent’s relationship with you, the dependent’s name, gender, date of birth, etc.

• You can add more dependents by clicking on the add button.
• Then you will have to provide your bank account information on the new page.

• Here you have to fill in the beneficiary’s bank name, branch name, account number, IFSC code, account type, etc.

• Then on the new page, you will have to upload any of your identity proof like Aadhar Card, Employment/Retirement Proof, Self/Family Photo, or Canceled Bank Cheque, etc.
• Then you will have to verify your Aadhaar, under which you will have to fill in your Aadhaar number and click on the Get OTP button.

• Now an OTP will come on the mobile number linked to your Aadhar card, which you will have to fill in here.
• Now finally you have to fill in the name of the beneficiary and click on the I agree button.

• Then you have to click on the Submit For Approval button.

• To make a claim under this scheme, you will first have to visit the official website of the scheme from the link given.
• Then you will have to fill in your employment type, PAN number, mobile number, etc. by clicking on the Click Here button and clicking on the Proceed button.
• Then you will get the dashboard of the scheme where you will have to click on the option named Reimbursement.

• Then you have to click on the Submit New Claim button.
• Then on the new page, you will have to fill in the name of the patient, date of admission, result of treatment, date of discharge, etc.

• Then you have to fill in the name of the hospital, district, and state and click on the Save button.
• Then on the new page, you will have to fill in the hospital type, pincode, hospital address, and registration number.

• You can also add more hospitals by clicking on the Add Hospital button, whose information you will have to provide here.
